Waiver of Responsibility:

 

You have discussed having an elective ultrasound with your healthcare provider.  You understand and acknowledge that the purpose of the ultrasound is for prenatal fetal bonding and keepsake images only.  

 

You understand and acknowledge that the information provided during your visit with Bella 3D Imaging is limited and does NOT take the place of an OB visit with your healthcare provider.  Bella 3D Imaging does NOT assume any responsibility of diagnosing, tracking, or reporting of any complications or medical findings of your pregnancy that may have been detected by a full diagnostic medical ultrasound. Meaning, you fully understand that the purpose of your visits with Bella 3D Imaging is solely for your personal leisure and is 100% non-diagnostic. You understand that all visits should be followed up with your OB healthcare provider.

 

You understand and acknowledge that sonograms can be limited due to many variables such as amount of fluid, fetal positioning, and maternal body habitus.  Because of this, pictures of your baby are not a guarantee. In the event this occurs, Bella 3D Imaging provides a courtesy one-time follow up visit that has to be utilized within the first two weeks of initial appointment. This does not go for pregnancy confirmations and packages purchased at a discounted rate. You agree that all sales are final.

 

You understand and acknowledge that Bella 3D has a zero tolerance for lateness.  If you are late, you agree to surrendering your appointment and rescheduling; if unable to reschedule you agree to keeping your original appointment and paying an additional $25 late fee. Cancellations are non-refundable, however if you cancel your session 24 hours or more in advance, we will issue a full refund. 

 

To ensure the efficiency of our session times and best serve all of our clients, full payment is required to confirm your booking. Payment can be made with Zelle/QuickPay/Venmo. This policy allows us to dedicate our full resources to your scheduled session and minimize scheduling conflicts. If we do not hear back from you OR payment is not received by the date listed in your confirmation email, your reservation may be declined. If unable to pay in full at time of booking, please let us know.

 

By agreeing to these terms and conditions, you are willing to comply and proceed with scheduling your 3D/4D session with Bella 3D Imaging.
